If you don't know what the Recap Video is I suggest you go investigate old Games. Anyway, if you play a tribute, please fill out the following form ASAP, since I'll be starting work on this immediately!

NOTE: PLEASE do not wait until your tribute dies to post this information. I do the picture editing right away, so if I have to wait for your tribute's picture I'm going to assume you don't have one and then I'll find one myself, and it's quite a hassle when I get a picture after that from you because you were waiting to post it until you died. So don't do it, just post all this information as quick as possible, and edit accordingly once/if you do die.
